Abstract: 

Successful long-term management of a chronic illness can be challenging for anyone, but for women leaving jail or prison, the problems of caring for themselves can be overwhelming. Health care providers can help with this by teaching patients how to obtain the skills they need to navigate the health care system, access services, and find high-quality care. The first step is motivating them to be healthy and see how important they are in their own health care.
