Abstract: 

The Hampden County Correctional Center has established a comprehensive public health model of health care for inmates. The model consists of five essential elements which are necessary for an optimum program of correctional health care. The individual elements are: 1.Comprehensive health assessment, 2.A community standard of health care and treatment, 3.Health education, 4.Prevention measures and, 5.Effective discharge planning that links the inmate post discharge from the community from which they came and to which 98% will return. This article specifically addresses the last element: discharge planning for patients with chronic or severe illnesses with the goal of maintaining continuity of care for inmates following discharge.
