Abstract: 

Patients with a history, health appraisal, or health evaluation that reveals chronic illnesses and who are remanded to prison will be incarcerated for a period of time long enough to warrant being enrolled in a chronic illness clinic. Chronic illness clinics are designed to treat and control known health care problems at a primary care level with the intent of keeping them from escalating into secondary and tertiary issues. This article surveys the main features that should be part of any correctional chronic illness clinic and includes a sample Chronic Illness Clinic Form, a simple, single-page form designed to facilitate continuity of care.
