Abstract: 

A report in the May 2016 issue of the BMJ by Makary & Daniel, “MedicalError – the third leading cause of death in the US” put a renewed focus on error reporting in health care. This article examines strategies for incorporating error reporting into a correctional health care system’s regular CQI program, with a special emphasis on developing non-punitive mechanisms for reporting errors, fostering a positive attitude in management with respect to error review, and creating a knowledge base of error types and risks.
