Abstract: 

In August of 2007, the position of discharge planning (DP) coordinator was created by the Washington County Jail’s (WCJ’s) contracted medical provider, Prison Health Services (PHS), and was approved for funding by the county. The position was designed to be fi lled by a masters of social work and supervised by the health services administrator. The discharge planner would function as a part of the facility’s mental health team. The primary objective of the DP coordinator would be to facilitate the medical discharge of inmates being released into the community and their re-entry into medical or psychiatric treatment services.
